Ms. Wanda, an active and sociable 82-year-old Caucasian female, spent her career at NASA and enjoyed a vibrant social life. However, her life took a challenging turn eight years ago when she was diagnosed with Chronic Obstructive Pulmonary Disease (COPD). This chronic condition led to frequent hospitalizations every 3 to 4 months due to COPD exacerbations, urinary tract infections (UTIs), falls, or dehydration.

For a long time, her daughter served as her primary caregiver, having quit her job to provide the necessary support. Unfortunately, Ms. Wanda’s condition continued to deteriorate, necessitating her move to a local rehabilitation hospital for physical therapy following a hospitalization for a UTI. Just before her discharge, she suffered a fall at the rehab facility, resulting in a cracked rib, and she arrived at HOPE in significant pain and with oxygen dependency.

Initial Assessment

Upon her arrival at HOPE Residential Assisted Living, our team conducted a thorough assessment of Ms. Wanda’s condition. She was weak, in pain from her recent rib fracture, and required continuous oxygen support.   • Zero Hospitalizations:
